Phone Service Maintenance Contract
Section One
Paideia School is seeking proposals from qualified telecommunications providers to furnish maintenance for a Mitel FX-200 ICP site with approximately 130 IP-5300 series phones and approximately 140 virtual extensions. Voice mail is on a vertical base First Class/Open Text mail system. Service should be available 24/7 365 remote and on-site, include unlimited training, discount for purchase of equipment not covered by the maintenance contract, and an annual audit and system health check. A hardware inventory list can be provided upon request. The contract was awarded in November of 2011 with a start date of July 1, 2012, for a period of 36 months.
Section Two
Your proposal must include the following:
Explain how support calls are placed with your company and routed to the Network Support Person.
State average response time for telephone support, non-critical on-site calls and critical on-site calls.
Define normal business hours and how emergency calls are handled outside of normal work business hours and holidays.
Include Company history and Resumes' of Support Personnel assigned to this contract.
Include any educational discounts and discounts for paying on a quarterly or annual basis.
Give account references that include the network components described in Section One. Provide other information explaining your companies qualifications.
Explain network equipment provided for connection to the school's local area network.
Section Three
Proposal must be valid for a period of not less than thirty (30) days.
If labor is requested, a valid Certificate of Insurance must be available and submitted prior to Commencement of work, if proposal is accepted.
Prices must include overhead and profit percentage for change work order.
If proposal is for materials only, shipping and/or freight changes not included are to be so noted. If no notation is made, shipping and/or freight charges will be considered as included in price.
